A Health Home is a community-based care management service model where all of a child’s caregivers and providers communicate with one another so that all social, academic, physical and behavioral needs are addressed in a comprehensive manner.

Health Home Care Management for Children

Health Homes help families navigate the complex care and social services systems, resulting in improved health and wellbeing for every child through individualized, quality care coordination. This is done primarily through a care manager who oversees and provides access to all of the services a person needs to assure that they stay healthy, out of the emergency room and out of the hospital. Care Managers continually communicate and share information with families/caregivers from providers in ways that are meaningful to them, sometimes acting as translators. Health Home services also ensure care during transitions from inpatient to other settings and promote ongoing medical and behavioral care to prevent crisis. Ultimately, Health Homes focus on securing the long-term success for every child and their family.

As a care management agency, Heritage Christian Services has contracted with the Children’s Health Home of Upstate New York (CHHUNY) to provide comprehensive care management in the following New York counties: Erie, Niagara, Genesee, Monroe, Ontario, Livingston, Orleans, Wayne and Wyoming. Who is Eligible for HHCM

Eligibility for Health Home Care Management

Children from birth to age 21 must be enrolled in Medicaid AND meet eligibility criteria by having two or more chronic health conditions (list of chronic conditions) OR one of the following single qualifying conditions:

  • HIV/AIDS
  • Serious Emotional Disturbance
  • Complex Trauma
  • Sickle Cell Disease

Potential enrollees must also demonstrate that they would benefit from Health Home Care Management Services. Heritage Christian Services has experts on staff that will work with you to determine if a child is eligible.
