We have partnered with the statewide fiscal intermediary, Public Partnerships LLC (PPL), to make sure you continue receiving the best care from your trusted personal assistants. As a facilitator, we are here to provide you with excellent customer service. You can reach out for support with enrolling/registering, technical assistance and other questions. How to Register with PPL In order for new consumers to select Heritage Christian as your facilitator, you must first register with PPL by using one of these options: Register by calling 1-833-247-5346 Create a PPL log in and register online with your phone number by clicking HERE. Create a PPL log in and register online with your email by clicking HERE. Join a weekly PPL virtual registration session by clicking HERE. Once registration is complete, you must call PPL at 1-833-247-5346 to request Heritage Christian Services as your facilitator. Personal Assistants must also complete registration with PPL AFTER your consumer has registered and complete the following: Fill out, sign, and submit the necessary forms and documents (online through the PPL system or in-person at the Rochester Heritage Christian Services office.) Complete a health assessment (only if expired.) Complete the necessary PPL trainings. Our CDPAP support specialists are available to schedule virtual appointments and host weekly in-person registration sessions for consumers and PAs in need of assistance. She is now 30 and making remarkable strides in her recovery. Toni expresses her gratitude for Tammy, her Heritage Christian Services client specialist. “Tammy guided me through the whole thing [PPL transition], answered all my questions, and ensured everything went smoothly. We were well-prepared, and I’m very thankful for her help,” Toni says. What Is Consumer Directed Personal Assistance Program (CDPAP) This service provides an array of options for long-term care. Consumers, or their designees, can hire family members, friends, neighbors or other people over 18 they trust to be an attendant. In many cases, people already providing these supports can now be paid for doing them. Spouses and designated representatives CANNOT serve as CDPAP assistants. An attendant can perform all the tasks usually assigned to a home health aide, such as assisting with bathing, eating, dressing and bathroom needs. An attendant can also perform tasks that nurses usually perform, such as administering medication, cleaning wounds, suctioning tracheotomies, giving insulin injections and administering oxygen to a person on a ventilator. CDPAP is overseen by the NYS Department of Health. Eligibility Must be a NYS resident Must qualify for Medicaid Can be an older adult or senior, a child, a person with a physical disability or intellectual and developmental disabilities, or anyone who requires assistance with daily activities or requires skilled nursing care Must be medically stable and capable of overseeing own care or have a designated representative.
