These services may include:Nursing Oversight and ServicesDurable medical equipmentHome and vehicle modificationsAdaptive TechnologiesRespite Care and CDPASOngoing Case Management and AdvocacyCoordination with schools, extracurricular programs and summer campsCrisis Intervention resourcesMedicaid coverage of medical equipment, provider costs, copays, prior approvals, transportation to medical appointments, etc.Eligibility for Care at HomeTo be eligible, a child must meet all of the following requirements:Be under 18 years of ageHave a developmental disability as defined by NYS’s Mental Hygiene LawDemonstrate complex healthcare needs that are expected to last longer than 12 monthsBe eligible for the level of care provided by an ICF or certified nursing home for persons with Developmental DisabilitiesNot be in a hospital or other nursing facilityBe ineligible for Medicaid because of parental income and resourcesBe able to be cared for at less cost in the family home than in an ICF facilityBy enrolling in the Care at Home waiver program, the child will be eligible for Medicaid, and may qualify for certain other state and federal services.Heritage Christian currently offers this program in Monroe, Livingston, Wyoming and Ontario counties.
